Discharge Medicines Review

Have your medicines changed after a hospital stay? Our pharmacy team can check your discharge information, help make sure your repeat medicines are updated correctly and answer any questions about what you should now be taking. This is a free NHS Wales service for eligible patients.

Support with your medicines after leaving hospital

Medicines are sometimes started, stopped or changed during a stay in hospital or another care setting. A Discharge Medicines Review helps make sure these changes are understood and carried through safely once you return home.

Our pharmacy team can compare your discharge medicines information with your current repeat medicines, identify possible discrepancies and contact the appropriate healthcare team when clarification is needed. We can also talk through your medicines and arrange follow-up support where appropriate.

How we can help

Review the medicines information provided when you leave hospital or another care setting
Check which medicines have been started, stopped or changed
Compare these changes with your current repeat medicines
Raise discrepancies with the appropriate healthcare team
Explain what your medicines are for and how to take them
Arrange follow-up support where appropriate

Who may be eligible?

You may be eligible if you have recently left hospital or another care setting and your medicines have changed, you take several regular medicines, or the pharmacy team believes that a review would help you. Eligibility is confirmed by the pharmacy team using NHS Wales service criteria.
